School safety information reporting system of Yipu high school

According to the "measures for the safety management of primary and secondary schools and kindergartens" (Decree No. 23 of the Ministry of education of the people's Republic of China),
schools should establish a complete system of students' safety information notification, so as to eliminate all kinds of potential safety hazards and deal with all kinds of accidents calmly.
Combined with the actual situation of our school, the following safety information notification system is specially formulated:

1、 Student attendance information bulletin:

(1) The teacher in charge of a class must establish the contact address book of the class, including the detailed home address of the students and the relatively stable contact telephone
number.

(2) Parents should be informed of the school work and rest time at the beginning of each semester. Parents should be informed in time when the schedule needs to be adjusted in a long
or short term due to the alternation of seasons or various activities organized by the school. The form of notice can be written notice to parents and short message group to ensure that the
notice is in place.
(3) Parents should be informed in advance of the rest and curriculum arrangements for winter and summer holidays, may day, Tomb Sweeping Day, Dragon Boat Festival, national day and
Mid Autumn Festival holidays. Parents should be informed in writing in advance and the receipt should be kept.
(4) The teacher in charge of a class and the teacher should be strict with the attendance system

In case of special circumstances such as abnormal absence or leaving school without authorization, the head teacher must report to the head teacher in time; the head teacher should
investigate the reasons for the students' absence as soon as possible and contact their guardians. If the guardian can not be contacted, or the situation is serious, the head teacher must
report to the level department and the relevant department of the school in time, and make corresponding records for future investigation.

(5) In case of abnormal absence or leaving school without authorization, which has major safety risks and parents cannot be contacted by telephone, they can visit their parents at home
or contact their parents through neighbors, classmates and other means of communication, and make corresponding records.

(6) During the class, the teacher of the class shall urge the class leaders to record the names of the absent students in the specified log or attendance sheet, and report to the head teacher
at the end of class.

(7) Strictly implement the school work and rest time, and shall not advance or postpone without authorization. If a student needs to be detained in school due to heart to heart talk or student
problems, the parents of the student must be informed in time and the matter of returning home shall be handled properly.

(8) If the head teacher finds that there are abnormal physical and psychological conditions of students and other information endangering the safety of students in their daily work, they should
inform their guardians in time and report to the level department or relevant departments. The level department shall establish corresponding confidential information files for tracking and
reference. If the situation is serious, report to the headmaster's office in time.

(9) The head teacher should check whether there are students with special constitution, specific diseases or other abnormal physiological and psychological conditions in the class. If so, they
must contact with their guardians, and inform the relevant departments of the level department and the school in a timely manner. The Department and the school doctor must record the safety
information of these students and keep the health and safety information of the students properly. For those students who are known to have special physique, specific diseases or abnormal
psychological conditions, and some activities that are not suitable for these students to participate in, the head teacher should give them proper attention and care, and privately tell them that
they can not participate, and at the same time, the personal privacy of students must be protected according to law.

2、 Student accident information bulletin

(1) In case of poisoning, epidemic of infectious diseases, safety accidents, criminal incidents and abnormal deaths of teachers and students, the relevant departments of the school shall deal
with and report to the superior authorities in a timely manner.

(2) The school should take measures to rescue the injured students according to the existing conditions and abilities, and send the injured to the nearby hospital as soon as possible, and
contact the parents of the students in time.

(3) The school set up an investigation team to quickly collect the relevant accident information, make corresponding records, and pay attention to the preservation of the accident scene and
relevant evidence.

(4) The content of poisoning incident report mainly includes: time, place, number of people poisoned, cause of poisoning, main symptoms, etc. The report of infectious disease epidemic events
mainly includes: the time and place of the first case, the number of infected people, the main symptoms, the preliminary diagnosis of health and medical institutions, etc. The reports of safety
accidents and abnormal deaths of teachers and students mainly include: time, place, number of casualties, cause of accident, treatment, etc.

(5) The information in the report must be true records. For the information that can be published publicly after being confirmed and reported to the relevant departments for approval, the time
and number of the published facts must be accurate; the cause of the accident should be announced according to the information mastered by the school.

(6) The unclear facts will be announced after investigation by the relevant parties

The privacy of students and families must be protected according to law. Relevant departments are responsible for the analysis, summary and filing of injury accidents and handling work.

(7) At the same time, the level department and accounting office shall report to the insurance company in time, and cooperate with the insurance company to settle the claim.

3、 Level by level reporting system for emergencies:

(1) If there is a student safety accident in the class, the head teacher, the teacher or the teaching staff on site should report to the Department and the relevant department of the school
immediately. It is not allowed to conceal, lie or omit to report, so as to ensure that the school can concentrate its efforts on rescue in the first time, and the school should report to the relevant
departments at the higher level at the same time.

(2) Strictly implement the personnel on duty to ensure the smooth and orderly operation of safety and emergency information reporting channels.

(3) The report procedure of safety emergency in school: Duty Officer - leader in charge - principal. Reporting should be timely, accurate, comprehensive, without omission and false report. The
contents of the report include: time, place and brief information of the accident.
